The Case Manager (RN) utilizes clinical expertise, discretion, and independent judgment to assess patients’ needs and coordinate care post-acute care in order to provide for a safe discharge. The Case Manager (RN) works in collaboration with the physicians, nurses, ancillary staff, and community agencies to identify and arrange for appropriate care. Resource utilization is considered when coordinating the patients’ needs through the continuum of care. The Case Manager (RN) practices a work-style reflecting the mission, vision and values of the Case Management team, as well as the Organization. 


  • Responsible for developing and implementing discharge plans for patients; this includes assessment of clinical, psychosocial, and financial needs of the patient and family.
  • Coordinates with the interdisciplinary team to gather pertinent clinical information in order to develop a comprehensive discharge plan.
  • Has knowledge of clinical course of patient’s condition in order to appropriately identify post-acute care medical and psychosocial needs.
  • Assist the physicians and care team with decision making on transfers for clinically complex patients to higher level of care and will liaison clinical information to the next care provider.
  • Will oversee the post-acute care arrangements for patients on ECMO or home ventilators.
  • Has knowledge of payer requirements for discharge.
  • Communicates and coordinates with resources in the community to meet the clinical and psychosocial needs of the patient for a safe discharge. This includes providing patient information within the guidelines of HIPPA .
  • Educates the patient on community and government resources to assist the patient with developing short/long term care plans.
  • Assist patient to advocate for self to receive appropriate services.
  • Documents pertinent patient issues, contacts and discharge plans in the medical record.
  • Responsible for appropriate utilization of resources during and post hospitalization.
  • Participates in achieving metrics established by the hospital to meet CMS quality standards, HCAPS, Magnet and other hospital initiatives.
  • Is a mandated reporter for elder, child, and spousal abuse.
  • Performs other duties as assigned.



  • Current license to practice as a Registered Nurse (RN), in good standing, within the State of California
  • Current American Heart Association Healthcare Provider BLS (CPR) certification
  • Self-directed individual with ability to prioritize multiple tasks and meet deadlines, excellent attention to detail, and maintain a high level of work quality.
  • Strong team player with excellent oral, verbal and presentation communication skills.
  • Proven ability to interface with all customer levels (physicians, Organizational staff, insurance companies, patients and family members).
  • Working knowledge of Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint, plus healthcare database systems.
  • Decisive and capable of exercising good judgment under pressure.
