Summary of Position

Under general supervision, coordinates the population management activities at a practice. Provides data management and patient outreach for specific patient populations with chronic illnesses, such as diabetes, hypertension or cardiovascular disease, or to broader patient populations due for other types of screening tests. Generates reports and makes inferences on data collected. Presents performance metrics on chronic diseases to clinicians and ensures clinicians are informed in a timely fashion. Collaborates with other sites' Population Management Coordinators to share findings and improve processes. Attends monthly Population Management meetings. Coordinates activities with Central Internal Medicine and organizational quality initiatives. Participates in care improvement activities as needed to improve quality performance. Typically reports to a department manager.

Minimum Requirements

Education: High School degree (or equivalent education) required. College degree preferred.

A bachelor's degree in appropriate discipline (typically in health services, business administration or a healthcare related field) or a non-clinical degree combined with an interest in healthcare and excellent problem-solving, multi-tasking, technology-literate skill sets can at times be substituted for work experience.

Skills and Experience: Skills and experience typically acquired through three years in a clinical business support setting. Previous training or experience in data management of chronically ill patients preferred. EMR experience and/or aptitude to master the EMR based on other technology experience required. Proficiency with Microsoft Office especially Excel is required. Proficiency in data management, including the ability to extract data and develop spreadsheets, required. Knowledge and understanding of medical terminology strongly preferred.

This position requires organizational, interpersonal, communication, writing, presentation, and problem solving skills in order to work cooperatively and effectively with all levels of personnel and to interact with patients. Excellent phone skills, strong customer service, and the ability to build relationships with patients by phone are required.
