The scope of Oncology social work "includes clinical practice, education, advocacy, administration, policy, and research" (AOSW Oncology Social Work Standards of Practice). The Hematology-Oncology licensed independent clinical social worker (LICSW) possesses knowledge about cancer and its treatment which complements and enhances the ability to provide psychosocial assessment and intervention for patients experiencing distress related to cancer and its treatment, as well as assistance in crisis situations. Performs psychosocial assessments and provides supportive counseling in the Ambulatory setting, and uses individual consultation, family, or group methods as indicated. Works both independently and collaboratively with the clinicians, nursing and support staff, and practices within the guidelines of professional social work ethics and standards.
* Completes a comprehensive assessment of patients including race, ethnicity, religion, culture, language, physical or mental disability, socioeconomic status, sexual orientation, gender identity or expression, and psychosocial status, including their functional limitations, strengths, and deficits.
* Performs a psychosocial assessment of the patient across the cancer continuum including new diagnosis, treatments, survivorship, and Stage IV disease/end-of-life using standard, approved methodology.
* Evaluates completed patient Distress Questionnaire results and assesses any patient with a Distress Score >7 to determine plan of care.
* Assesses the patient's support system.
* Develops a treatment care plan with patient and family input including individual therapy, psychoeducation, or counseling with appropriate and competent interventions
* Anticipates and identifies psychosocial issues that may impede progress or act as a barrier to care and recommends interventions to the clinician and the team.
* Assists patients and families to understand the psychosocial factors to maximize utilization of cancer related community resource utilization.
* Assists patients, families, clinicians, and staff in understanding significant social and emotional factors related to cancer diagnosis, treatment, disease progression, and end-of-life care.
* Consults with clinicians and staff around patient communication strategies or crisis intervention, and to identify appropriate pathways for patients needing concurrent behavioral health treatment.
* Collaborates with and provides psychosocial consultation to the multidisciplinary care team to promote a smooth, coordinated plan of care and to assist patients and families in reaching an informed decision about their care. Actively participates in the development and delivery of Survivorship programs and activities in support of patients and care-givers.
* Fosters a collaborative, team-focused work environment to achieve goals.
* Identifies, develops, and champions ideas for new/expanded programs in specific program areas to promote new or improved patient services.
* Maintains accurate and timely records of services rendered to patients, using Progress Notes and Problem List to document assessment, interventions, and reassessment of patient/family response.
* Recommends policy and/or practice changes to enhance departmental operations. Actively engages with Practice management to implement improvements.
* Plans for own continuing professional development and supervision using resources within Atrius Health or other agencies.
* May supervise and mentor social work students and interns.
* Performs other duties as assigned.
* Social Work students or interns as arranged and approved.
* Master's degree in social work from an accredited institution.
* Current license to practice Social Work (LICSW) in Massachusetts or Eligible.
* Minimum of 3 years post master's degree social work experience and previous bachelor's level related experience in a medical setting.
* Three years of Oncology social work experience required.
