Under the supervision of the Diabetes Clinical Nurse Specialist and Program Manager provides education to patients, families and staff regarding management of diabetes to maintain health and prevent other complications they may be associated with clinical conditions such as heart disease, cancer, pregnancy and other conditions that diabetes is known to effect.

Assesses patient care needs based on the primary condition of the patient which drives the plan (ie. Pregnancy related diabetes; type 1 versus type 2; other related conditions such as heart disease or cancer.

Independently assess individual insulin and oral diabetes medication needs based on glucometer and other lab results;

Teaches patient and/or family regarding insulin injection techniques for administration by self or others;

Evaluates adherence to prescribed diet and exercise program, and recommends adjustments to patient care plan;

Collaborates with other disciplines as indicated to develop appropriate plan and education for patient.

Educates patients and families as needed to ensure patients can safely and independently manage their diabetes at home to maintain stable glucose levels.

Collaborates with dietitian as indicated to ensure thorough education for diabetes management.

Assesses knowledge gap and need for continuing follow-up about knowledge of diabetes care.

Ensures an excellent customer experience providing timely care and adjusts teaching plans for patient and family in accordance with assessed needs; or as directed by provider staff.


2-3 Years Experience with the clinical care of diabetes patients; this can include inpatient or outpatient medical management of patients
